Over 8 years of experience completing evaluations and treatment documentation for workers' compensation cases.

How Evaluations Are Conducted

Evaluations follow DWC standards and the AMA Guides to the Evaluation of Permanent Impairment, Fifth Edition.

Request Evaluation

Contact Dr. Kapoor's office by phone or email to discuss the case and schedule an evaluation appointment.

Records Submission

Submit relevant medical and dental records for review prior to the scheduled evaluation date.

Evaluation

In-person dental and orofacial evaluation conducted at the nearest office location.

Report

Written report typically completed within approximately 14 business days, depending on case complexity and receipt of complete records.
